Appointments

We see our patients on an appointment basis, and ask that you please schedule in advance so that we may reserve a time for you. Appointments may be scheduled by calling our office. There is a list of our offices in this brochure along with their address, phone, and fax numbers.

We make every attempt to see you at the scheduled time, without any delays. At times when an emergency may cause a delay in seeing your doctor, we will let you know. If you are unable to wait, we will be happy to reschedule your appointment at a time that is convenient for you.

If you are a new patient, you willbe asked to complete a patient information form along with your insurance information prior to your appointment. Please bring your insurance card(s) with you.

The appointment line is open from 8:00am, Monday to Friday, and a receptionist will be able to assist you.



Alternatively you can book your appointment online. There are fixed (10 minute) doctor appointments which are either 'Pre-Booked' or 'Same-Day' appointments. Same-Day appointments are saved to be booked on the day only.
